Policy governing use and disclosure of personally identifiable information (PII) and personal health information (PHI)

What is the purpose of collecting PII/PHI? Wisconsin Health Insurance Advocate, LLC collects personally identifiable information (PII/PHI) for the purpose of helping you to enroll in the health insurance plan of your choice. You have the right to request restrictions on the use of your information. Your disclosure of PII/PHI is optional; however, broker may not be able to assist you with plan enrollment or trouble-shooting without your disclosure of PII/PHI.
What is collected? PII/PHI includes your full name, SSN, date of birth, credit card information, marital status, immigration status, income, race/ethnicity, and more. This information is collected solely for the purpose of enrolling in a health insurance plan or when broker provides trouble-shooting assistance with the plan.
To whom will broker disclose PII/PHI? Your PII/PHI will only be disclosed to federal and/or state agencies and to the insurance company to which you apply and any applicable subcontractors for the purpose of enrollment. Any subcontractors are fully licensed agents who are subject to the same rules and HIPAA guidelines. Your information may also be provided to a Field Maintenance Organization for the purpose of completing or confirming your plan enrollment/retention, payment of commissions, and other confidential business purposes. Your information will not be released without express permission by broker or Wisconsin Health Insurance Advocate, LLC to any parties not listed above unless required by law. If you wish for your information to be released to any third party (including adult children or caretakers) you must give Wisconsin Health Insurance Advocate, LLC specific instructions to do so.
How long will my PII/PHI be stored? Your personal information is kept confidential and will be securely stored for the length required by law and in accordance with insurer requirements. This time varies from 3 years to 10 years depending upon the type of policy. After the termination of your policy, your information will be securely stored or destroyed as required by Wisconsin and Federal law and in accordance with your insurance company’s requirements, and upon the end of that period the information will be shredded or otherwise destroyed.
Can I get a copy of what information is stored? Yes, you have the right to inspect and request a written copy of your personal information stored by broker and Wisconsin Health Insurance Advocate, LLC. In accordance with applicable law, you may be required to pay the actual cost of providing written copies including employee time.
Can I request a change to my information? Yes, you have the right to request amendment to your information. Prior PII/PHI will continue to be stored but will be noted as amended.

You have the right to request copies of any information stored during the retention timeline.
